Any how-to links on how to rebuild the driveshaft? Parts list?

I've been having a vibration at any speed, doesn't matter if I'm in gear or coasting in neutral, accelerating or not. Vibration is less noticeable at higher speeds though. I got under my truck yesterday and noticed the carrier bearing was ****ed at a 10-15 degree angle and I could press it up and down pretty good bit. So I've decided to rebuild the entire drive shaft. Do I need anything other than the carrier bearing and the 3 u-joints?

Also, on the carrier bearing...is it just he bearing that gets replaced or the cover too?
