1. I have a question regarding the order form (regular Gryphon, for a 2006 F-150). On page three where the three drop downs for "level 1, 2 and 3 calibrations" appear, Do those boxes apply if no custom tune is desired?

2. I plan to order via regular mail using a check.
a. How do I indicate the "holiday special" number on the order form?
b. What will the shipping/any other fees be? (36330 ZIP in SE Alabama)

Any other info I might need to send the correct amount by check?
