Both you and mmandley correct about both 12 and 18 volts being needed. Ford uses an 18V signal on one of the DLC connector pins to initiate programming:

"FEPS: Flash EEPROM Programming Signal. 18 volt DC signal sent by the scan tool to initiate PCM reprogramming."

Since there's only a nominal 12V from the battery, I would assume that Gryphons and the like have a DC-DC converter to up 12V to 18V for that one pin during PCM programming.
