TDE0 and TDE1 are virtually identical. The sticker indicates that you were reflashed to TDE1 (almost 9 years ago!!) so that settles those questions. It makes little difference as we'll use TDE1 on the chip anyway.
The order of programs is strictly a matter of personal preference. We do always recommend running stock in the first position as this is the position the chip defaults to if there is ever a problem with the switch. The order you have indicated is what I would have recommend anyway. Yes... please include the order form with the M/O. Make the M/O out to William Cohron. Hope that helps.
